I do not accept in-network insurance. I will provide you with a monthly statement, called a superbill. If you have out of network benefits, you can submit the superbill to your insurance for reimbursement. It is your responsibility to check with your insurance company to ask if superbills are accepted and how much is reimbursed. You may be asked for CPT codes. The code for the initial intake appointment is 90791. The code for all future appointments is 90837. Some insurance companies also may cover future appointments under 90834, so be sure to ask about both codes.
